Analiza uticaja augmentacione mamaplastike na funkciju kože dojke kod žena

Uvod: Augmentaciona mamaplastika je u većini država najčešća estetskaoperacija, sa tendencijom stalnog porasta. Komlikacije augmentacije dojki sumoguće i različite, ali je veoma malo literaturnih podataka koji se odnose napromene fukcije kože posle ove operacije.Cilj rada: Osnovni cilj rada je bio da ispita da li estetska augmentacionamamaplastika može da izazove značajne promene funkcije kože dojke kodzdravih žena. Sekundarni cilj je bio da se ispita da li su te promene povezanesa veličinom dojki pre operacije, veličinom implantata, starosnom dobi igenerativnim periodom.Ispitanici i metodologija: Rad je urađen kao opservaciona-analitička,prospektivna, kontrolisana studija otvorenog dizajna, kod 128 zdravih žena.Kod 64 žene koje nisu rađale, starosti od 18 do 60 godina, sa definisanimisključujućim kriterijumima, urađena je obostrano estetska augmentacionamamaplastika. U kontrolnoj grupi je bio isti broj ispitanica. Pre i posleoperacije ispitivano je osam parametara funkcije kože: senzibilitet zadodir, diskriminacija dve tačke, osećaj za toplo, osećaj za hladno, osećaj zabol, temperatura kože dojke i funkcija znojnih i lojnih žlezda dojke.Rezultati: Posle augmentacije dojki dolazi do promene svih osam parametaraistraživanja. Osećaj za dodir i diskrimincija dve tačke su veći kod mlađihosoba i u generativnom periodu. Osećaj za toplo i hladno opada sa godinama,veličinom dojki pre operacije i veličinom implantata. Prag osećaja bola jeniži kod žena u generativnom periodu i onda kada su korišćeni manjiimplantati. Posle operacije nastupa porast temperature kože dojke, ali bezznačajane razlike u odnosu starost, generativni period, veličinu dojki iveličinu proteze. Funkcija znojnih žlezda dojke opada sa veličinom dojki preoperacije i veličinom implantata, a funkcija lojnih žlezda dojke je bolja kodmlađih osoba i onih koje su u generativnom periodu. Rezultati ispitivanjasvih ostalih navedenih parametara su slični i za levu i za desnu dojku.Zaključak: Većina rezultata iz ove studije su novi. Ova studija pružamogućnost da se urade i detaljnija i dodatna istraživanja svih parametara izstudije. Pri tome, mogle bi da se urade studije i sa drugim vrstamaimplantata, drugim hirurškim rezovima i mestima plasiranja implantata,kao i kod pacijentkinja koje su rađale. Takođe, bilo bi korisno da se uzme uobzir i debljina kože i indeks telesne mase. Bitan značaj ove studije zahirurge i pacijente je preoperativno informisanje pacijenata o mogućimpromenama funkcija kože dojke posle augmentacione mamaplastike.Objektivno preoperativno informisanje je važno obzirom na globalnu težnjuka očuvanju kvaliteta života pacijenata i stanovništva.

Introduction: Augmentation mammoplasty is the most common aesthetic surgery inmost countries, with constant increase. Complications of breast augmentation arepossible and different, but there are not many searches about changes of skinfunction after this surgery.Aim: The primary goal of this study was to investigate the effects of aestheticaugmentation mammoplasty on breast skin functions. The secondary goal was toexamine relations of these changes with preoperative breast size, implant size, ageand generativne period.Material and methods: Observational-analytical, prospective, controlled study ofopen design included 128 healthy women. Bilateral aesthetic augmentationmammoplasty was performed in 64 women that had never given birth, aged 18 to 60years, with well defined exclusion criteria. The control group included the samenumber of non-operated women. Before and after the surgery, eight parameters ofskin function were examined: sensitivity to touch, two-point discrimination, feelingwarm to the touch, feeling cold to the touch, feeling pain to the electricity, breast skintemperature and function of sweat and sebaceous glands of the breast.Results: Compared with the findings of breast skin function before surgery, as well asthe findings in the control group, after breast augmentation there is a change in alleight parameters of the study. The sensitivity to touch and discrimination of twopoints are higher in younger people and in the generative period, but they do notdepend on the size of the breast before the surgery and the size of the implant. Thefeeling warm and cold to the touch decreases with age, the size of the breast beforethe surgery and the size of the implant. The pain threshold is lower in women in thegenerative period and when smaller implants were used. After the breastaugmentation, there is an increase in the temperature of the breast skin, but without asignificant difference between age groups, generative period, breast size and the sizeof the prosthesis.The function of the sweat glands of the breast decreases with thesize of the breast before the surgery and the size of the implant, and sebaceous glandfunction is better in younger patients and those in the generative periodThe results ofthe all other research parameters are similar for both the right and the left breast.Conclusion: Most of the results from this study are new. This study provides anopportunity for more detailed and additional researches of all parameters from thisstudy. In addition, studies could be performed with other types of implants, othersurgical incisions and implant placement sites, as well as in patients who have givenbirth. Also, it would be useful to consider skin thickness of the breast and body massindex. The greatest significance of this study for surgeons and patients ispreoperative informing patients about possible changes in breast skin function afteraugmentation mammoplasty. Objective preoperative information is important due tothe global aspiration to preserve the quality of life of patients and the population.
