Efekat terapijske izmene plazme kod pacijenata sa različtim neurološkim poremećajima

INTRODUCTION: The main aim of this study is to determine the efficacy of therapeuticplasma exchange in patients with neurological disorders.METHODOLOGY: The study will be conducted on a sample of 22 patients withneurological disorders, who were hospitalized at the Clinic of Neurology, University ClinicalCenter Kragujevac, in the period from December 2019 to December 2022. In them,therapeutic plasma exchange (TPE) was the last therapeutic option, and it was applied whenthe patients did not respond as expected to standard pulse corticosteroid therapy. To test theefficacy, clinical improvement and laboratory tests (complete blood count, hemostasis statusand observation parameters specific to certain diseases) will be monitored. Based on thismonitoring, the degree of efficacy to be tested will be determined.EXPECTED RESULTS: The results of the study will reveal whether the outcome of thedisease will be better in patients treated with therapeutic plasma exchange. The parametersmonitored in therapeutic plasma exchange will be analyzed in order to compare them with theclinical outcome of TPE. These parameters are access (peripheral vein or central venouscatheter), amount of plasma removed, amount of substitution, amount of blood processed,amount of anticoagulant consumed, fluid balance, volume of exchanged plasma, duration ofthe procedure and ultimately the success of the procedure itself.EXPECTED CONCLUSION: The conclusion should provide an answer to whether the useof TIP improves the general condition as well as laboratory parameters in neurologicalpatients. The assessment of therapeutic efficacy will certainly be affected by the incompleteknowledge of the etiopathogenesis of most neurological disorders, the simultaneous use ofimmunosuppressive therapy, and the lack of simple laboratory tests for the quantification ofthe “pathogenic substrate” in the patient’s blood and removed plasma.
UVOD: Glavni cilj ovog rada je utvrđivanje efikasnosti terapijske izmene plazmekod pacijenata sa neurološkim poremećajima.METODOLOGIJA: Studija će se sprovesti na uzorku od 22 pacijenta saneurološkim poremećajima, koji su hospitalizovani na Klinici za neurologiju UKCKragujevac, u periodu od decembra 2019. godine do decembra 2022. godine. Kod njih jekao poslednja terapijska opcija bila terapijska izmena plazme (TIP), i primenjena jeonda kada pacijenti nisu očekivano odreagovali na standardnu pulsnukortikosteroidnu terapiju. Za ispitivanje efikasnosti pratiće se da li imakliničkog poboljšanja i laboratorijske analize (kompletna krvna slika, hemostaznistatus i parametri posmatranja specifični za određene bolesti). Na osnovu ovogpraćenja odrediće se stepen efikasnosti koja će se ispitivati.OČEKIVANI REZULTATI: Rezultati ispitivanja otkriće da li će ishod bolestibiti bolji kod pacijenata koji se leče terapijskom izmenom plazme. Analiziraće separametri koji se prate kod terapijske izmene plazme kako bi se i oni uporedili sakliničkim ishodom TIP-a. Ti parametri su pristup (periferna vena ili centralnivenski kateter), količina uklonjene plazme, količina supstitucije, količinaprocesuirane krvi, količina utrošenog antikoagulansa, bilans tečnosti, volumenizmenjene plazme, vreme trajanja procedure i na kraju sam uspeh procedure.OČEKIVANI ZAKLjUČAK: Zaključak bi trebalo da pruži odgovor da li seprimenom TIP-a postiže poboljšanje opšteg stanja kao i laboratorijskihparametara kod neuroloških pacijenata. Na procenu terapijskog učinka će svakakouticati i nepotpuno poznavanje etiopatogeneze većine neuroloških poremećaja,istovremena primena imunosupresivne terapije i nedostatak jednostavnihlaboratorijskih testova za kvantifikaciju „patogenog supstrata“ u bolesnikovoj krvii otklonjenoj plazmi.
