Kvalitet života operativno lečenih pacijenata u odnosu na konzervativno lečene pacijente sa hroničnim pankreatitisom

Introduction: Study compares quality of life and intensity of pain in patients with chronic pancreatitis who had surgical or conservative treatment.Materials and Methods: A study was performed on 50 patients that were divided into two groups, each with 25 patients: group of conservative-nonsurgically treated patients and a group of surgically treated patients. Comparing of quality of life in patients with chronic pancreatitis was performed using validated standardized questionary EuroQol-5 dimension (EQ-5D-5L ) and validated standardized questionary for pain intensity assessment Short form McGill Pain Questionnaire (SF-MPQ). Both of these are multidimensional questionaries that provide qualitative and quantitative assessment of pain intensity and quality of life. Patients filled in questionaries based on determined criteria for each questionary and statistical data analysis was performed using PАSW Statistics v.18 software.Results: Results have shown that patients with chronic pancreatitis that were surgically treated had a better quality of life compared to those treated with conservative therapy. It has also been confirmed that patients with chronic pancreatitis that were surgically treated had experienced lower intensity of pain compared to those treated with conservative therapy. Usually performed surgical procedures were Frey procedure 8 (32%) and Roux and Y procedure , 8 (32%), making combined almost 2/3 of all preformed procedures 16 (64%). There were found statistically significant differences between surgically and conservatively treated patients in all 5 dimensions of EuroQol-5 dimension (EQ-5D-5L). There is also statistically significant differences between afferent and sensory component of SF-MPQ questionary between two groups of patients.Conclusion: Quality of life in patients with chronic pancreatitis is better in those who had surgical treatment than in patients that received conservative- nonsurgical treatment.
Uvod: Studija upoređuje kvalitet života i intenzitet bola operativno lečenih i konzervativno lečenih pacijenata sa hroničnim pankreatitisom.Materijal i metode: Studija je obuhvatila ukupno 50 pacijenata, koji su podeljeni u dve grupe, sa po 25 pacijenata: grupa konzervativno-neoperativno lečenih pacijenata i grupa hirurški lečenih pacijenata. Upoređivanje kvaliteta života pacijenata sa hroničnim pankreatitisom je vršeno uz pomoć validiranog standardizovanog upitnika EuroQol-5 dimension (EQ-5D-5L) i validiranog upitnik za procenu jačine bola Short form McGill Pain Questionnaire (SF-MPQ). Radi se o multidimenzionalnim upinicima putem kojih se dobija kvantitativna i kvalitativna procena kvaliteta života i intenziteta bola. Pacijenti su popunjavali upitnike po već utvrđenim kriterijumima za svaki upitnik pojedinačno, a statistička analiza podataka, dobijenih nakon popunjavanja upitnika, je urađena u statističkom kompjuterskom programu PASW Statistics, verzija 18.Rezultati: Rezultati su pokazali da su pacijenti sa hroničnim pankreatitisom, koji su operativno lečeni, imali bolji kvalitet života u odnosu na pacijente lečene konzervativnom terapijom. Takođe je utvrđeno da hirurški tretirani pacijenti sa hroničnim pankreatitisom imaju manji intenzitet bola, u odnosu na pacijente koji su konzervativno tretirani. Najčešće izvođene hirurške procedure su Frey procedura, 8 (32%) i Roux and Y procedura, 8 (32%), čineći zajedno dve trećine svih procedura 16 (64%). Pronađene su statistički značajne razlike između hirurški i konzervativno lečenih pacijenata u svih 5 dimenzija EuroQol-5 dimension (EQ-5D-5L) upitnika. Postoji statistički značajna razlika aferentne i senzorne komponente SF-MPQ upitnika između dve praćene grupe pacijenata.Zaključak: Kvalitet života pacijenata koji boluju od hroničnog pankreatitisa je bolji posle operativnog lečenja u odnosu na pacijente koji su konzervativnoneoperativno lečeni.
