Ispitivanje morfoloških karakteristika nazopalatinalnog kanala i povezanosti sa horizontalnom dimenzijom kosti prednjeg dela gornje vilice primenom kompjuterizovane tomografije konusnog zraka

Uvod: Nazopalatinalni kanal predstavlja dominantnu anatomsku strukturu u regiji prednjeg dela gornje vilice. U literaturi se navode brojne komplikacije usled perforacije nazopalatinalnog kanala prilikom ugradnje implantata, stoga je ispitivanje morfoloških karakteristika nazopalatinalnog kanala i okolne kosti visoko preporučeno upreoperativnoj analizi.Cilj: Ova studija imala je za cilj da ispita povezanost oblika nazopalatinalnog kanala i dimenzije kosti prednjeg dela gornje vilice kao regije od interesa za imedijatnu ugradnju implantata.Materijal i metode: Studija je uključila 113 CBCT snimaka. Korišćenjem sagitalnog preseka ispitivan je oblik nazopalatinalnog kanala, kao i njegove morfometrijske karakteristike. Takođe je ispitivana horizontalna dimenzija kosti prednjeg dela gornje vilice na četiri nivoa. Aksijalnim presekom utvrđena je dimenzija incizivnog otvora.Rezultati: Zabeleženo je najveće prisustvo levkastog oblika, zatim oblik cilindra, oblikpeščanog sata, a najmanja zastpljenost bila je banana oblika (40, 35, 28 i 10, redom). Uočena je značajna razlika u dimenziji nazalnog i incizivnog otvora između različitih oblika nazopalatinalnog kanala. Dimenzija kosti je bila značajno manja na B, C i D nivoima kodispitanika sa banana oblikom nazopalatinalnog kanala u odnosu na ispitanike sa drugim oblicima.Zaključak: Kako odabir dimenzije i oblika implantata direktno zavisi od dimenzija kosti, posebnu pažnju u planiranju imedijatne ugradnje implantata u regiji centralnih sekutića treba posvetiti pacijentima sa nazopalatinalnim kanalom oblika banana koji imaju najmanju horizontalnu dimenziju kosti.

Introduction: The nasopalatine canal is the most prominent anatomical structure in the region of the anterior maxilla. Numerous complications are reported in the literature caused by perforation of the nasopalatine canal during implant placement, well the examination of the morphological characteristics of the nasopalatine canal and the surrounding bone is highly recommended to be analyzed.Aim: This study aimed to examine the relationship between nasopalatine canal shape and anterior maxilla dimensions as a region of interest for implant placement.Materials and methods: The study included 113 CBCT scans. Using a sagittal section, the shape of the nasopalatine canal, as well as its morphometric characteristics, was examined. The horizontal bone dimensions was also examined at four levels. The incisive foramen was determined by axial section.Results: The most common nasopalatine canal shape was the funnel, followed by cylindrical shape, hourglass shape and banana shape (40, 35, 28, and 10 respectively). A significant difference in the dimensions of the nasal and incisive foramen was observed between the different nasopalatine canal forms. The bone dimension was significantly smaller at the B, C and D level in subjects with banana-shaped nasopalatine canal compared to subjects with other forms.Conclusion: As the size and implant shape selection depends on the bone dimensions, special attention in planning the implant placement in the maxillary central incisors region need to be inpatients with banana-shaped nasopalatine canal who had the smallest horizontal bone dimension.
