Introduction: An unusable transfusion unit is a unit of blood or blood preparation obtainedfrom blood that does not meet European quality standards for the use of safe blood andcannot be administered to a patient who needs it. There are several factors that affect andcontribute to the unusability of transfusion units and they are: (1) inadequate education andselection of blood donors, (2) improper way of taking and processing blood units, (3)improper way of storing and storing transfusion units, (4) units of blood positive for markersof blood-borne diseases, and (5) improper preparation and administration of individual bloodunits.Аim: To determine the factors related to the unusability of blood units and blood derivativesin transfusion practice.Material and methods: The study is designed as an observational, cross-sectional study,which analyzes all collected blood units in the transfusion service in a certain period of time,to identify units of blood and its derivatives that are unusable, as well as the reasons whythey became unusable. The control group consists of units of blood and blood derivatives thatwere usable according to European quality standards and constitute safe or secure blood. Thestudy is retrospective and was conducted in the organizational unit of blood transfusion inBar for a period of two years - starting from July 1, 2016 to July 1, 2018.Results: A significant percentage of analyzed blood units taken from donors were not usable(11.0%), with the most common reasons being: expired (36.9%), chylous blood (26.8%),insufficient blood volume (14.8%), hemolysis (10.6%), damage to the packaging during theprocess of taking, processing and preparation of blood (8.1%), blood positive fortransmissible diseases (3.4%), blood suspected of transmissible diseases (8.7%) and thepresence of clots in erythromass (0.6%). The percentage of unusable blood was higher whenblood was donated during the winter than during other seasons. Blood sampling from donorsoutside the health facility, and especially in schools, was associated with a higher frequencyof unusable transfusion units. When taking blood from donors with comorbidities, thefrequency of unusable transfusion units was significantly higher than when taking blood fromdonors without comorbidities.Conclusion: Only safe blood is a drug that is of high quality, effective and safe in thetreatment of patients who need it. Adherence to European standards on safe blood and theintroduction of the necessary preventive measures, especially when there are risk factors,significantly reduces the number of unusable transfusion units, which contributes to fasterand better treatment of patients.

Uvod: Neupotrebljiva transfuziološka jedinica predstavlja onu jedinicu krvi ilipreparat krvi dobiven iz krvi koji u traženom momentu ne zadovoljava evropskestandarde kvaliteta o primeni bezbedne krvi i ne može se primeniti pacijentu komeje ona potrebna. Postoji više faktora koji utiču i doprinose neupotrebljivostitransfuzioloških jedinica: (1) neadekvatna edukacija i selekcija davalaca krvi, (2)nepravilan način uzimanja i obrade jedinica krvi, (3) nepravilan načinskladištenja i čuvanja transfuzioloških jedinica, (4) jedinice krvi pozitivne namarkere krvlju prenosivih bolesti, i (5) nepravilan način pripreme i primenepojedinih jedinica krvi.Cilj: Utvrditi faktore koji su povezani sa neupotrebljivošću jedinica krvi iderivata krvi u transfuziološkoj praksi.Materijal i metode: Studija je dizajnirana kao opservaciono istraživanje po tipustudije preseka, gde su analizirane sve prikupljene jedinice krvi utransfuziološkoj službi u određenom vremenskom periodu, kako bi se otkrilejedinice krvi i njenih derivata koje su neupotrebljive, kao i razlozi zbog kojih supostale neupotrebljive. Kontrolnu grupu čine jedinice krvi i derivata krvi koje supo evropskim standardima kvaliteta bile upotrebljive i čine bezbednu ili sigurnukrv. Studija je retrospektivna i sprovedena je u organizacionoj jedinici transfuzijekrvi u Baru u periodu od dve godine - počevši od 1. jula 2016. godine do 1. jula2018. godine.Rezultati: Značajan procenat analiziranih jedinica krvi koje su bile uzete oddavalaca nije bio upotrebljiv (11,0%), pri čemu su najčešći razlozi bili: istekaorok trajanja (36,9%), hilozna hrv (26,8%), nedovoljan volumen krvi (14,8%), hemoliza(10,6%), oštećenje pakovanja pri procesu uzimanja, obrade i pripreme krvi (8,1%), krvpozitivna na transmisivne bolesti (3,4%), krv sumnjiva na transmisivne bolesti(8,7%) i prisustvo ugrušaka u eritromasi (0,6%). Procenat neupotrebljive krvi jeveći kada se krv donira tokom zime, nego tokom drugih godišnjih doba. Uzimanje krviod donatora van zdravstvene ustanove, a posebno u školama, je povezano sa većomučestalošću neupotrebljivih transfuzioloških jedinica. Kod uzimanja krvi oddonatora sa komorbiditetima učestalost neupotrebljivih transfuziološkihjedinica je značajno veća nego kada se krv uzima od donatora bez komorbiditeta.Zaključak: Samo bezbedna krv predstavlja lek koji je kvalitetan, efikasan i siguranu lečenju bolesnika kojima je ona potrebna. Poštovanje evropskih standarda obezbednoj krvi i uvođenje za to potrebnih preventivnih mjera, posebno kada postojefaktori rizika, značajno smanjuje broj neupotrebljivih transfuzioloških jedinicašto doprinosi bržem i kvalitetnijem lečenju obolelih osoba.
